Certification

Pag i-isyu ng sertipikong kopya ng kapanganakan,kasal at kamatayan.Legitamation of Illegitimate Children,Aplikasyon ng lisensya sa kasal at Naantalang rehistrasyon ng kapanganakan.

Ang civil registry ang siyang may hawak sa pagdodokumento ng mga sertipikong kagaya ng sa kapanganakan, kasal at kamatayan ng isang tao, ito ay maaaring makuha sa pamamagitan ng paghiling ng isang sertipikadong transcript of photocopy mula sa opisina.
WHO MAY AVAIL: 18 y/o and above resident of the Municipality of Malasiqui

For Birth Registration

Required Documents:

  • Municipal Form 102 duly accomplished
  • Marriage Certificate of parents (if parents are married)
  • Affidavit to use the surname of the father (if parents are not married)

For Death Registration

Required Documents:

  • Municipal Form 103 duly accomplished/Municipal Form No. 103A (for fetal death)
  • Burial permit
  • Certificate of transfer of cavader (if deceased is to be buried outside of Malasiqui)

For Marriage License

Required Documents:

  • Municipal Form 97 duly accomplished
  • Affidavit of Cohabitation, Certificate of No Marriage issued by the PSA, Birth Certificate of contracting partiesd and Birth certificate of children (if contracting parties married under Art. 34 of the New Family Code)

Late Registration of Birth

Required Documents:

  • Negative Certification issued by PSA
  • Affidavit of two(2) disinterested person who might have witrnessed of have known about the birth of the child
  • Sworn Statement of the present whereabouts of the mother if the person seeking late registration of on illegitimate child is not the mother.
  • Certificate of Marriage (if married)
  • CEDULA (Community Tax Certificate)
  • Any two (2) of the following documents evidences which may show the name of the child, date, place of birth and name of parents such as:
  • Any two of the following documents showing the child's name, birth details, and parents' names: Baptismal Certificate, Form 137, Insurance Policy, Parents' Income Tax, or Medical Records.

Legitimation of Illegitimate Children

Required Documents:

  • Certificate of Live Birth issued by Philippine Statistics Authority.
  • Certificate of Marriage (Cenomar) of parents/Marriage Advisory Philippine Statistics Authority
  • Certificate of Marriage of Parents issued by Philippine Statistics Authority
  • CEDULA (Community Tax Certificate)
  • Affidavit of two (2) disinterested person
